A physician must refer patients to the cardiac and pulmonary programs. To refer to these programs, call 404.501.7155 or fax the referral form to 404.501.7689. Call us or fax patient’s name, diagnosis, home phone number and which program you want them in. We will then contact the patient to discuss our program, set up an orientation appointment and give them directions to our facility.
The following diagnoses are appropriate for cardiac or pulmonary rehabilitation:
We can also help patients with coronary artery disease, hypertension, pulmonary restrictive diseases and cardiomyopathy.
Medicare will cover several diagnoses. Currently, they will cover 80% of the cost of cardiac rehab for a patient with MI (within a year), CABG, stable angina, angioplasty, stent and transplant. Most insurance will cover 70% to 100% of the cost of cardiac or pulmonary rehab if the physician deems it medically necessary.
Wear comfortable, loose fitting sports attire such as shorts or jog pants, athletic shoes and a t-shirt or polo shirt.
When you are new to rehab, we will arrange with you a time for you to come in to be evaluated and to get your initial program set up. After that, you may work out during the operating hours of The Wellness Center, which are:
Monday - Friday: 5:30 a.m. - 9 p.m.
Saturday: 8 a.m. - 4 p.m.
Sunday: 11 a.m. - 5 p.m.
Depending on what your doctor has requested for your particular situation, exercise sessions may last 30 minutes up to an hour at a time. Please allow plenty of time during your visits before and after exercising for changing clothes, applying and removing monitors, blood pressure checks, bathroom breaks, etc.
Yes. The Wellness Center stamps your parking ticket from the two surface lots and the parking deck. Also, there are ten parking spaces reserved just for rehab patients in the lower surface lot. Read More information about driving, parking and walking directions.
Your doctor will advise how soon you may start coming to rehab. Once we have your doctor’s instructions and insurance information, we will contact you to set up your first appointment.
